At Lebanon Trail High School, my goal is to promote the health, safety, and well-being of every student while supporting them in staying at school, in class, and ready to learn. When students are healthy and supported, they are better positioned to succeed academically and build a strong foundation for lifelong health and achievement.
If your student has a medical condition, health concern, or requires health-related support during the school day, please complete the appropriate health forms. Once completed, forms may be submitted to the Trailblazer Clinic in person, by email, or by fax.
Parents/Guardians: It is your responsibility to determine whether your student is well enough to attend school each day before arriving on campus. The school clinic is not an entry point for medical care and does not replace evaluation or treatment by your student's healthcare provider. If your student requires medical evaluation or treatment, please contact their healthcare provider or seek appropriate medical care.
The goal is simple: healthy students, successful learning, and a safe school community.

You Should Stay Home from Work or School When
· Have had fever (100°F/38°C or higher) within last 24 hrs
· You've been keeping fever down by taking medication within the last 24 hrs
· Have been vomiting within last 24 hrs
· Have had diarrhea within last 24 hrs
· Other contagious symptoms like persistent coughing
· Unusual rashes
· Pink eye
· Not being able to focus and/or participate meaningfully
· Your symptoms make you a risk to yourself or others
When to Be Extra Cautious

If you have close contact with others at school or work.
• Vulnerable Populations:
If you are around children, the elderly, or immunocompromised individuals, it's even more critical to stay home. Your illness could be transmitted to someone at home via those you have interacted with today.
Prevent spreading illness and allow your body to rest and recover.

Dear Trailblazer Parents and Guardians,
As we enter the holiday season, the school clinic has experienced an increase in visits from students with common cold symptoms. Since there is no cure for these minor illnesses and the school nurse cannot diagnose or treat them, students with mild symptoms—such as a runny nose, mild cough, nasal congestion, or occasional sneezing—should remain in class or stay home if they are not feeling well enough to participate in learning. Students who do attend school can usually manage these symptoms while practicing simple self-care, including using tissues, drinking water, covering coughs and sneezes properly, and maintaining good hand hygiene.
If your student has had a fever of 100.0°F or higher or has experienced vomiting or diarrhea in the last 24 hours, they should stay home.
It is the responsibility of parents and guardians to assess whether their child is well enough to attend school each day before arriving on campus. If your child requires medical evaluation, we encourage you to contact their healthcare provider directly.
Parents may excuse up to four absences per semester by reporting them through the school website. If your child sees a doctor, dentist, or utilizes telehealth services, please provide a note to the attendance office so the absence can be officially excused.

Update on SB 12 as of 10/06/2025 Routine Health Services Consent
Senate Bill 12, which became effective law on September 1, 2025 requires school districts to seek parental consent from parents for health services provided to students. After receiving clarification from the Texas Education Agency (TEA), school districts can seek parental consent under Senate Bill 12 through a selective opt-out option. This means that moving forward, parental consent for routine services is presumed unless a parent chooses to opt-out of a particular routine health services offered by the school district.
