If you would like for your child to keep Tylenol, Motrin, Benadryl, or Tums in the clinic please fill out this form. Your child can have 10 doses of these approved over-the-counter medications. A Doctor note must be obtained after the 10 doses. Any other over-the-counter medication MUST have a doctor’s signature on this form.  

Parents need to supply any medication to the school nurse.
