Districts policies:
Children's Ibuprofen (Advil, Motrin) -- ibuprofen can be the only active ingredient
Children's Acetaminophen (Tylenol) -- acetaminophen can be the only active ingredient
Benadryl -- diphenhydramine can be the only active ingredient
Tums
All other over-the-counter medication MUST be accompanied by a Doctor's note for the nurse to give while at school
NOTES:
Parent/Guardian supplies all medications
A completed and signed PARENTS REQUEST FOR ADMINISTRATION OF MEDICATION BY SCHOOL PERSONNEL form must accompany medication
Parent should bring age-appropriate medication to the school nurse in the original, properly labeled container; expired medications cannot be administered
The school nurse may give up to 10 doses of the above-listed district approved OTC medications
After 10 doses have been administered, the parent/guardian will be notified that a physician's note is needed to continue to administer that medication to the student
All other oral over the counter medication must have a doctor’s note
All medication must be in the original container & must be properly labeled. The parent/guardian must complete & sign the Request for Administration of Medication form & the parent/guardian should bring the medication to the school nurse.
If your child needs to carry their own inhaler, epi pen or Cystic Fibrosis medication, then please complete the:
SELF-ADMINISTRATION OF PRESCRIPTION ASTHMA OR ANAPHYLAXIS MEDICATION BY STUDENTS
All prescription medication that cannot be scheduled for other than school hours may be given at school.

Medication should be in the form prescribed by the child's physician (if the dose is a half of a tablet, please send in the medication already halved)
Parent/Guardian must bring the medication to the school nurse, do not send medication with child.

Medication should match the description on the bottle ( example: peach, oval-shaped with 833 imprint)
Prescriptions must be picked up by Parent/Guardian by last day of school or it will be disposed of according to FISD policy
