This is a great opportunity for your 5th grader to learn outside of the classroom. Teachers, admin and a nurse will be accompanying your child on this adventure. The packet will be given by your child's teacher and will need to be filled out entirely before your child is able to attend.
These medications are permitted WITHOUT a physician's signature (but still require parent's/guardian's signature):
acetaminophen (Tylenol): acetaminophen must be the ONLY active ingredient
ibuprofen (Advil, Motrin); ibuprofen must be the ONLY active ingredient
diphenhydramine (Benadryl): diphenhydramine must be the ONLY active ingredient
If you plan to send in these medications, they must be listed on the Camp Medication Form. All dosing will be based on package directions.
ALL OTHER OVER-THE-COUNTER MEDICATIONS MUST BE ACCOMPANIED WITH PHYSICIAN'S SIGNATURE OR ORDERS!

Hello parents,
If your child is going to 5th grade science camp, please turn in your child’s medications on or before _____. All medications brought must be included in the medication permission form for the field trip signed by the parent. If medication requirements for the trip are not met, medication will not be administered. Any meds dropped off after the designated drop off date will be turned away unless spoken to me first.
· SENDING MEDICATION: All medication (prescription and over the counter) must be in a labeled container (original box/bottle or with prescription label). Any loose pills or medicine will not be accepted
· You need a doctor’s note for ALL medication, even if it is over the counter (except for Tylenol, Ibuprofen, Benadryl, and TUMS)
· If you wish for me to take any medications from school, you must let me know.
· If your child is taking a medication to be given with a nebulizer, you must send the tubing, cup, mask, or mouthpiece with the medicine needed (in labeled box with the prescription label or signed note from doctor).
· If your child has an inhaler but “never” needs it, send it anyway with the prescription label or signed note from doctor because we may need it in case of an emergency
