Welcome to Ashley Elementary School! We are so excited to have your child in our Kindergarten class next year! Please furnish the school a current shot record from your doctor or health clinic as soon as possible so we can complete the enrollment process and finish the registration for your child. We are not able to complete the registration process without this record and your child cannot be placed in a classroom or assigned a teacher until the shots are current and up-to-date. Please see below regarding immunization requirements.

New Kindergarten Parents:
Upcoming kindergartners should be well on their way to caring for their own bodies. Bathroom skills are of the utmost importance! Kinder kids must be able to use the restroom themselves, wipe themselves, and wash their hands.
Don't forget to pack an extra set of clothes and underwear in their backpacks everyday in the event of a potty accident and/or spills. Children will be expected to clean up and change themselves if an accident occurs.
Required Immunizations K- 12 grade:
Dtap- 4/5 doses, 1 dose after 4 years old
Polio- 3/4 doses, 1 dose after 4 years old
MMR- 2 doses, 1st dose after 1st birthday
Hep A- 2 doses, 1st dose after 1st birthday
Hep B- 2 doses, usually 1st dose at birth
Varicella- 2 doses,1st dose after 1st birthday or had the disease

Temperature of 100 degrees or above (must have temperature below 100 degrees for 24 hours before returning to school WITHOUT taking a fever reducing medication).
If your child vomits- DO NOT SEND THEM TO SCHOOL. They need to be able to tolerate at least 2 meals with no vomiting before returning to school-even if they don't have a fever. Your child may be sent home from school for vomiting one time.
Children with diarrhea (3 or more episodes of loose stools in 24 hours) cannot come to school until they are diarrhea free for 24 hours without the use of diarrhea suppressing medications.
Pink Eye/Conjunctivitis: Your child cannot come to school unless it has resolved and/or with doctor's note.
Signs of severe illness, including fever, irritability, difficulty breathing, crying that doesn't stop with the usual comforting, or extreme sleepiness.
Questionable rash until a physician has determined the rash is not caused by an infectious disease.
Known communicable disease must be treated for the appropriate amount of time as directed by a physician.
Recommendation of a physician or school nurse.
Lice: Student must receive one treatment of medicated lice shampoo and no active lice are present before returning to school. School nurse must check student before entering classroom. For more info: http://www.cdc.gov/lice
Don't forget to send an email or written note to the office to excuse your child from being absent (phone calls and verbal notification do not count- we must have documentation). If the absence exceeds 3 days, then a doctor's note is required.
