Students who are interested in an ELO should submit this form independently or with the help of their guidance counselor.

Meet with the ELO Coordinator

Schedule a time to meet with the ELO Coordinator to begin setting up your ELO in your Personalized Learning Plan.

Complete ELO Page on the PLP

The ELO page of the PLP must be completed in full before the ELO can begin. This includes a description of the standards the student intends to work on, a Learning Log to track their learning throughout the ELO, weekly goals to provide benchmarks for when they plan to complete work, and a brief description of the final project. This page also contains the ELO Contract.

Once the page is complete, the ELO Coordinator will send a link to the page to the student's guidance counselor, the Cooperating Teacher, the student's adult at home, the Principal and a Community Mentor if necessary. All parties should review the plan and reach out to the student and ELO Coordinator with any questions or concerns. As each party accepts the plan, they should e-sign the contract with their name and date. All parties must agree to the ELO standards and content and the contact should be signed by all parties in the order that the parties are listed on the form. Once the contract has been signed, the ELO will be added to the student's schedule in Powerschool. To get a sense of how this might look, click here.

If an ELO student cannot connect with a content Cooperating Teacher, they can still complete the ELO. However, the credit associated with the work will be Elective credit.

During the ELO...

The student will:

  • Update Learning Log with evidence on a daily basis

  • Meet with the ELO Coordinator on a bi-weekly basis during AT or class

  • Develop their final project or product

  • Check in with their Cooperating Teacher promptly if they need support in understanding content.

The ELO Coordinator will:

  • Check-in with student on their progress regularly

  • Assist when necessary during AT or scheduled class time

  • Address questions and concerns of all parties, should they arise

The Cooperating Teacher and Community Mentor will:

  • Provide student with the content support they need during previously agreed upon times when requested by the student

Schedule Verification Presentation

Once the student has completed evidence for all of their identified standards and feels that they are ready to present their final project, they should email all the assessing parties to schedule a Verification Presentation.


Present

The student should formally present their ELO work to the Cooperating Teacher(s), ELO Coordinator, Community Mentor(s) if applicable, and any other necessary adults. At least three adults from the school community should be present.
