PERMISSION FOR SCHOOL ADMINISTRATION OF PRESCRIPTION MEDICATIONS
Prescription medications are medicines that require a written prescription from a health care practitioner. In order for a child to be given prescription medicine at school, the child’s health care practitioner and the child’s parent or guardian must sign a permission form. This form can be found ONLINE, or you may pick up a copy from the nurse’s office. A form must be submitted for each medication.
A parent or guardian should deliver the medicine and the permission form to the school nurse. The medicine must be in its original container with the pharmacy label on it. The label should list the child’s name, dosage instructions, and the name of the medication. We cannot accept medicines in plastic bags.
If a prescription medicine is needed while your child is on a field trip, you must bring a separate supply to school on the day of the field trip. It must be in the original, marked container. Additional labeled bottles may be obtained from your pharmacy. We cannot accept medicines in plastic bags.
