Every student and parent/guardian must sign a student agreement form before beginning their ELO.
This form will need to be completed before a student can begin their ELO.
What is FERPA?
The Family Educational Rights and Privacy Act of 1974 (20 U.S.C. §1232g; 34 CFR Part 99) is a federal law
that protects the privacy of student education records. Under FERPA, a school may not disclose personally
identifiable information from a minor student’s education records to a third party unless the student’s
parent/guardian has provided written consent.
Students will complete this plan with the ELO Coordinator prior to beginning an ELO.
The final reflection prompt is located in this plan.
Some students may be asked to record their time formally using this timesheet.
