Family One® Home Health Agency (HHA) provides skilled nursing services to people with disabilities of all ages, elderly (over 65 years) and people with chronic health conditions with activities of daily living, instrumental activities of daily living and observation and redirection of behaviors in patients' homes. 

Family One® Home Health Agency services, includes:

  • Wound Care & Dressing Changes

  • Chronic Disease Management (e.g., diabetes, hypertension, COPD)

• Therapeutic Services (e.g., physical, massage, occupational, and speech)

• Medical Social Service:

  • Counseling & Support

  • Financial Assistance Programs

• Personal Care Assistance (e.g., Homemaking, Light Housekeeping and Companionship)

• Respite Care for family caregivers

*An RN conducts a one-on-one personal care assessment for each client to develop a personalized care plan.
