Family One® Home Health Agency understands the importance of personal connections and word-of-mouth marketing, and we would like to reward our employees and the general public who make referrals to our agency with a small incentive.
Read below to learn more about program rules and guidelines.

Applies To: Employees, Independent Contractors, Community Partners, and Clients
The purpose of this Referral Program is to encourage and reward individuals who refer eligible clients and qualified staff to our home health agency. The program supports community engagement, staff recruitment, and improved client access to quality care services.
Eligible Referrers:
Active employees and independent contractors of the agency in good standing.
Current clients or family members of clients.
Community partners and healthcare professionals (where not restricted by conflict of interest or regulatory guidelines).
Ineligible Referrers:
HR personnel directly involved in hiring.
Marketing agencies or third parties under paid advertising agreements.
Individuals prohibited from participating under applicable laws or contracts.
Client Referral: A new client who:
Has not previously received services from the agency;
Completes intake and begins active services for at least 30 consecutive days.
Staff Referral: A new employee or contractor who:
Is not currently or previously employed by the agency;
Completes the full onboarding process and maintains active employment for 60 consecutive days.
To qualify, all referrals must be submitted in writing using the Referral Form (available through the agency office or website) before the referred individual applies for employment or receives services.
Each referral form must include:
Referrer’s name and contact information
Referred person’s name and contact information
Relationship between the referrer and the referral
Incomplete or late submissions may be disqualified.
Client Referral Reward:
A $25.00 gift card or cash equivalent once the client has received 30 days of service.
Staff Referral Reward:
$150 bonus or gift card once the new hire completes 60 days of employment.
(Amounts may vary based on position type, client program, or promotion period.)
Rewards will be processed through payroll or mailed within two pay cycles following eligibility verification.
Referrals must be genuine and made in good faith.
Multiple referrers for the same referral will result in the reward going to the first verified submission.
There is no limit on the number of referrals an eligible participant may submit.
The agency reserves the right to deny or revoke rewards if eligibility criteria are not met or if fraudulent activity is suspected.
Rewards may be subject to applicable taxes or reporting requirements.
All referral activities must comply with HIPAA, state regulations, and anti-kickback statutes.
Referrers may not offer or imply medical, financial, or insurance benefits in exchange for participation.
Client or staff information obtained during the process must remain confidential and used solely for referral purposes.
The agency reserves the right to modify, suspend, or terminate this program at any time, with or without notice. Any changes will be communicated through official agency channels.
