Family One® Care Services is excited to join the CDC (Centers for Disease Control and Prevention) National Diabetes Prevention Program provider network to support our clients, employees, and community members in reducing their risk of diabetes.  Through our hands-on lifestyle coaches and the evidence-based PreventT2 Curriculum, participants gets to support and tools in supporting them on their lifestyle change journey.

Family One® Care Services is a preliminary recognition CDC Diabetes Prevention Recognized (DPRP) Organization. “CDC-recognized organizations deliver evidence-based type 2 diabetes prevention programs in communities across the United States.
