  • Enter the Customer ID Code, 66EAE6EOB0. Click “Validate”, then click “Launch Baseline”.

3. Select English as the language, click “Next”.

4. Click “Next” on the following screen.

5. Enter your information as asked on the screen. Continue to move forward to the next screens answering the information being asked.

6. Click “Enter Additional Demographic Information”. Continue through this section entering information as asked.

7. Continue to Current Signs and Symptoms. When answering these, select “0/NOT EXPERIENCING” for all of them, unless you have chronic symptoms.

8. Once you have entered all of your information, you may begin the test. READ INSTRUCTIONS CAREFULLY AND THOROUGHLY.

9. When finished, you do not need to print or enter your email.
