Important Information

In order to participate in sports, including cheer and dance, student athletes must have the following -

1. A complete registration.

2. A signed medical eligibility form, aka a physical, from a doctor. These forms are valid for 2 years from the exam date.

3. An ASB sticker - available to purchase through Evergreen Payments or at the ASB window at school.

4. All fines and fees paid. Please contact the Athletics office if you need assistance.

5. A GPA of 2.0 or higher and no more than 1 failing class.

Students may not participate in practice or games without a current "Medical Eligibility Form" signed by a doctor.

Reminder: Accompanying Adult Required for Students at After School Events

All students must be accompanied by a responsible adult for all after school events. This includes any sports games, meets, or tournaments. If the responsible adult leaves early, then the student(s) must leave with them.
