Students may not participate in practice or games without a current "Medical Eligibility Form" signed by a doctor.

Important Information

In order to participate in sports, including cheer and dance, student athletes must have the following -

1. A complete 25-26 MS Athletic & Activities Registration Form. There are 5 steps. All 5 must be completed.

2. A signed medical eligibility form, aka a physical, from a doctor. These forms are valid for 2 years from the exam date.

3. An ASB card - available to purchase through Evergreen Payments or at the ASB window at school.

4. All fines and fees paid. Please contact the Athletics office if you need assistance.

5. A GPA of 2.0 or higher and no more than 1 failing class.

Instructions:

1. Go directly to registration through the following link: https://wa-evergreen.intouchreceipting.com 

2. Enter the parent’s Skyward Family Access username and the password you created to log in or follow the instructions to create or reset a password.

3. Click on the student’s name that you are registering and then click on “Register for Athletics and Activities” under the "Shop" section.

4. Choose the appropriate registration packet for your needs:

   1. 25-26 Shahala MS Athletics Registration (Interscholastic, competitive sports including cheer, dance, and knowledge bowl) – please select ALL sports your student might be interested in! Please complete ALL 5 forms - a registration is incomplete without them.

      1. 25-26 Informed Consent (Shahala)

      2. 25-26 Consent to Share Information (Shahala)

      3. 25-26 Transportation Acknowledgement (Shahala)

      4. 25-26 Parent/Guardian Athletic Code of Conduct Agreement (Shahala)

      5. 25-26 Athletics Registration (Shahala)

   2. 25-26 Shahala MS ASB Card Purchase - once per year

5. Please drop off the paper copy of the MEDICAL ELIGIBILITY FORM, aka a physical, to the Shahala ASB Office (after 08/11/25) or email a scanned version to us at Active_SHA@evergreenps.org

To Update a Registration/ Add a Sport:

1. Follow the instructions above to log in and get to the 25-26 Shahala MS Athletics Registration.

2. Click on "Complete"

3. Click on "Fill Out" next to Step 5: 25-26 Shahala Athletics Registration.

4. Make any changes to the sports section or whatever section you need to edit.

5. Scroll down and click submit.

You do not need to change Steps 1-4 unless any of that information changes.
