Payment and Insurance

Lake Park Counseling is In-Network with the following Insurance Providers:

• Blue Cross Blue Shield PPO (Illinois and reciprocal with all other US states).

• United Health Care PPO (Optum)

• USHIP (University of Chicago's insurance)

• Out of network insurance: LPC can provide you with a statement to be submit to insurance for possible partial session reimbursements, depending on your plan.

Out of Pocket Payment:

• Out of pocket fee for Individual Psychotherapy:

  • Masters Level Clinician, Independently Licensed (LCSW, LCPC, PhD)

    • $150 for diagnostic intake session

    • $130 for subsequent individual psychotherapy sessions. (See Services for more information.)

  • Masters Level Clinician, under Clinical Supervision (LSW, LCP)

    • $130 for Diagnostic Session

    • $110 for individual psychotherapy sessions.

• Out of pocket Couples and Family Counseling:

  • $150 for diagnostic intake session

  • $140 for subsequent couples and family counseling sessions. (See Services for more information.)

• Reduced fee options are available. Please let LPC know if you cannot use insurance and feel you need a reduced rate.

Lake Park Counseling accepts cash, check, HSA and all major credit cards.

We can provide bills, statements, or invoices for submission to your insurance company for reimbursement.
