PD Request Form: To be completed for professional development only. Professional development is defined as advanced professional learning intended to help administrators, teachers, and other educators improve their professional knowledge, competence, skills, and effectiveness. Allowable cost include substitutes, registration, travel, and teacher stipends. Use the PD rate of $65 for a half day and $105 for a full day and use $100 per day for a substitute.
SIP/SAP* Meeting Request Form: To be completed when requesting time to meet with school improvement team and/or teachers as related to the strategic planning process at the building level. Each building has been allocated $1,500.00 for the school year to be used for this purpose. Because the high schools are attempting to get AdvancEd accreditation this year, an additional $3,000 is available for each high school team.
Curriculum Work Request Form: To be completed in special circumstances in which the curriculum work is beyond the allotted instructional cycle committee meetings as planned at the district level. Completion of this form requires prior conversation with Instructional Leadership because funds are limited to very specific pre-approved purposed.
Please note that the teacher contract does not require additional payment to teachers for PD or curriculum work unless attendance is required. We have extended the practice to include purposes that we intend to promote because of district priorities or the availability of specialized grant dollars.

Conference Leave Request Form Make a copy of this form when asked to do so. Please type to complete the form. Once you have completed the form, print it out for the Employee and Principal/Administrator signatures that are REQUIRED before being sent to the Assistant Superintendent of Instruction. A typed signature is not acceptable and the form will be returned to you for actual signatures. When printing, all signatures MUST BE contained to the front side of one page.

Grant Submission Request A request for approval for the submission of a grant must be completed and provided to the Board of School Trustees for approval prior to grant submission. The Board of School Trustees deadlines should be used to determine deadlines for submission of the request. Failure to obtain necessary approval could result in an inability to submit the grant application. *In accordance with Board Policy 6110, ALL grant requests must be approved by the Board of School Trustees prior to grant submission. Failure to complete this process could result in an inability to accept awarded funds.
Elkhart Education Foundation grants do not require the completion of this form. Please ensure your administrator is aware of your grant application prior to submission.
Grant Submission CONFIRMATION This form must be submitted if Board Approval was not obtained prior to submission. Please complete this form and the Request for Grant Submission if you submitted a grant prior to Board Approval.
