In the Elk Grove Unified School District, we believe in partnering with the students, families/caregivers, and community agencies to support the long-term life goals of our students. These goals can include but are not limited to equal access to post-secondary educational opportunities, competitive integrated employment, and the opportunity to live with the greatest level of independence possible. We know that the sooner families/caregivers and students begin thinking and talking about life after high school, the better the outcome for the students.
With support from the California Department of Education (CDE) WorkAbility I program, EGUSD Transition Services looked at what information and supports would be most beneficial to students, families/caregivers, and staff with regard to supporting these long-term goals. This website serves as a clearinghouse of resources for the families/caregivers and staff of students with IEPs.
These single-page documents provide an overview of the variety of skill focus areas and steps important to the long-term success of our children with disabilities. Use them as guides in conjunction with the resources linked below.Â
