Duplicate Textbooks
Per Elk Grove Unified School District guidelines, a second set of textbooks may be issued if it is medically necessary for students who have a note on their doctors's office letterhead from a currently practicing medical doctor, chiropractor, or family nurse practitioner. Please see below for district guidelines on how to receive duplicate textbooks.
The note must be signed and the signature must match the name printed on the note/prescription.
An Acceptance of Responsibility form (see below for form) must be completed by the student, and parent/guardian assuming financial responsibility.
The issuance of additional textbooks will begin the second week of each term to ensure that all enrolled students get their first set of books.
Additional textbooks may be picked up in the library during normal school operating hours.
Because of the medical concern requiring the second set of textbooks, it is strongly recommended that the student be accompanied by the parent/guardian at the time of textbook pickup
The student will be responsible for both sets of books, including copies stored in the classroom.
Parents will be billed for any lost or damaged textbooks.
