Medication and Action Plan's
If you have noted down on your child's enrolment form that they have a medical condition that requires medication to be given we may require forms to be filled out. An ASCIA Action Plan MUST be signed by a doctor EACH YEAR for the following conditions:
Anaphylaxis
Allergies
FPIES
Eczema - does not require a GP's signature
Hayfever - does not require a GP's signature
Asthma Action plans must also be updated each year and signed by your GP. If your child can administer their medication by themselves, their medication can stay in their bag. If your child need's help, your child's spacer and inhaler can be stored in the First Aid room.
General reminder: Please note that it is the responsibility of the parent to provide an up to date copy of the ASCIA plan to the school prior to your student commencing with us. It is also the responsibility of the parent to:
provide the ASCIA action plan including an up to date colour photo of the student where necessary
inform the school if their child’s medical condition changes, and if relevant provide an updated ASCIA plan with a colour photo
provide the school with their child’s auto-injector and any other medications prescribed. i.e. antihistamines, puffers, panadol etc
replace the auto-injector and any other medications as prescribed on the ASCIA plan prior to expiry
supply alternative food options when required
supply a second auto-injector for camps and other school events if required after notification from the Principal/Assistant Principal
Your child's medication and action plan's can be dropped off to the Office on your child's first day of school
