Review this Choose Your Own Adventure in Forms: Haunted Halloween

Then create your own:

Step 1: Create Form: Name it Story Adventure

Step 2: Add a Question “Which story do you want to hear” - Multiple Choice with options Horror or Fantasy

Step 3: Enable ‘Go to Page Based on Answer’

Step 4: Create a section titled ‘Horror’ & another section titled ‘Fantasy’

Step 5: Select for each choice the page to go to

Practice Google Forms

Google Forms can be used to create questionnaires and collect feedback from a large number of participants. In this activity you need to gather anonymous input from your community.

Create a form titled 'Community Survey'
Add three questions
1. Do you think seniors should be able to have a 'flexible schedule?' (multiple choice)
2. What do you think the benefits are? (open response)
3. What do you think the drawbacks are? (open response
Make sure anyone can access the form
Only 1 response per person
Edit the confirmation message with this text 'Thank you for your input!'
Send the form via email and/or post on web.
