Step 1: go to this google form and register.  Select HOSA

Step 2: Fill out these 2 forms:

 Parent permission form linked here, AND the CTSO Student Handbook Form

I have copies of these outside my classroom #2060. (forms get turned into the bin in room 2060

Step 3: Pay fee.  Pay the fee on the website using a debit or credit card. There is a link in the google form that you takes you to pay.

Step 4: Bring the 2 forms from step 2 to classroom 2060. There is a bin to turn them into.
