NY Required Physical Exams

NYS REQUIRED PHYSICAL EXAM- GRADES K, 1ST, 3RD, 5TH & 7TH

NY State Health Examination form is the ONLY acceptable for we are accepting.  All pediatrician and health offices have been informed of this, but should you have an issue, the form is available on the link below.  If I do not receive an updated Physical Exam by May 2021, your child will be seen by our district physician, Dr. Coletta.
