Return to School Form

ATTENTION PARENTS,

If your child is sick, you will need to see your Health Care Provider (HCP) as per NYS Dept. of Health

• They determine whether or not COVID test is necessary

• You need Return to School (RTS) form for your child prior to coming to school after illness.

• CLICK HERE for form to bring to your child's HCP, as all HCP have a RIS form.
