Please complete the claim reporting kit and return it to Human Resources as soon as you can. Your supervisor must also complete a form in the kit (listed as part 3) and return it to Human Resources.
You will also need to complete the employee portion of the DWC-1 form and send that as well. We will complete all employer portions and return them to you to provide to your care provider.

What is Federal Work Study?
The federal work-study program provides jobs for a limited number of students with financial need, allowing students to earn a paycheck while in school. Students must complete a valid Free Application for Federal Student Aid (FAFSA) each year in order to apply for federal work-study. The federal work-study program supports off-campus jobs in community service and on-campus jobs related to a student’s course of study, in addition to positions that support ongoing work within university departments.
