Credential Program Forms

The following forms are available through Taskstream

  • Credential Requirements

    • All state requirements for the credential are listed and updated as needed for students. Students can access this information on their Taskstream page.

  • Classroom Observation

    • During Preparation for Supervised Student Teaching and Student Teaching semester, students will be observed by their University Supervisor a minimum of 10 times. The University Supervisor meets with the Teacher Candidate to provide constructive feedback on the student’s progress. Before each meeting, students are required to submit a Dominican Lesson Plan through Taskstream, outlining the lesson.

  • Intervention Action Plan

    • An Intervention Action Plan may be initiated as needed by the Program Director. The Program Director will work with faculty, staff, and students to set goals to help meet the professional and curricular requirements of the credential program.

  • Progress Monitoring Guide

    • Three-way conferences (among University Supervisor/Resident Mentor, Student Teacher/Intern and Directing Teacher/Mentor) provide classroom performance feedback and support for students in Supervised Teaching. The conference provides opportunities for sharing perspectives on the candidate’s teaching methods, for strengthening the feedback and support given to the student, and for providing direction for the next steps in improving teaching performance.

  • Final Student Teaching Evaluation

    • University Supervisors evaluate the student teacher's performance on the TPEs and 21st Century Skills and submit the final evaluation at the end of their placement.

  • Individual Development Plan (IDP) (Now included in Credential Requirements DRF)

    • Before exiting the preliminary program, candidates, district-employed supervisors, and University Supervisors collaborate on an IDP consisting of recommendations for professional development and growth in the candidate's clear credential program. The IDP follows the new teacher to their induction program.

  • Individualized Educational Plan (IEP)

    • Education Specialist candidates are required to participate in an IEP meeting during the Supervised Teaching portion of the program. Candidates will learn about the special education process, assessment, instructional planning, and Universal precautions. It is preferable that the candidate lead the meeting, although each district has its own policy in this regard. Candidates are evaluated, using this form, by their University Supervisor on their ability as an IEP meeting participant.

  • Directing Teacher Program Evaluation (Survey Link)

    • Directing Teachers provide feedback about strengthening our teacher preparation programs. A survey link are sent directly to the Directing Teacher towards the end of the academic year.

  • University Supervisor & Resident Mentor Program Evaluation (Survey Link)

    • University Supervisors and Resident Mentors provide feedback about strengthening our teacher preparation programs. A survey link are sent directly to them towards the end of the academic year.

  • Credential Program Evaluation (Survey Link)

    • Students provide feedback about strengthening our teacher preparation programs. A survey link are sent directly to the student.
