Submission Details
Eligibility:
Open to all Department of Health (DOH) Centers for Health Development (CHD) Planning Units.
Each CHD may submit one (1) or more best practice entry.
Submission Format:
Entries must use the attached Annex B: Best Practice Submission Template made accessible via this link https://tinyurl.com/PNM2025BPTemplate.
This includes the following:
Title of the Best Practice
Problem Statement
Objectives
Description of the Best Practice
Results and Evidence (quantitative and/or qualitative)
Sustainability Strategy
Completed Basic Information Section (contact details and signature)
Supporting documents and/or photos (optional but strongly encouraged)
Evaluation Criteria:
Entries will be rated based on the following criteria:
40% Relevance and Impact:
Addresses a clear problem and contributes meaningfully to UHC goals or planning improvements.
30% Innovation and Efficiency
Demonstrates a creative or improved approach that uses resources wisely.
20% Scalability and Sustainability
Has potential for long-term use and replication by other CHDs or units.
10% Clarity of Documentation
Clearly written, well-documented, and easy to understand.
Recognition:
Selected best practices will be featured during the 2025 Planners’ Network Meeting and will be shared for cross-learning with other CHD
