• Work Experience Sheet (Attachment to CS Form No. 212) 

• Certificate of Civil Service Eligibility, Board Rating, or Professional License

• Transcript of Records and Diploma;

• Training Certificates (if applicable); and

• Performance Rating (required for government employees, including Job Order and Contract of Service personnel) 

• Application documents should be in Portable Document Format (.pdf) with proper file name: Document Type_Last Name 

(examples: Cover Letter_Cruz, PDS_Cruz, TOR_Cruz)
