Our Community Health and Outreach Program is here to support people on the streets whenever they need us. Whether it’s a physical concern, a mental health struggle, or just questions about staying safe and healthy, we meet people where they are through street medicine practices. Our goal is to connect clients to higher levels of care when needed, or to help them get and stay healthy ourselves.
Once their concern is resolved, they can graduate the program with the ability to come back anytime if they need us again!
Street medicine is the practice of providing healthcare directly to people experiencing homelessness—on the streets, in camps, under bridges and inside shelters.
Instead of waiting for people to come to clinics or hospitals, street medicine brings care to them, meeting individuals where they are, with compassion and respect.
Street medicine programs address urgent health needs like infections, wounds, chronic illness, and mental health concerns. They also focus on prevention—helping people avoid unnecessary hospitalizations by treating conditions early.
At its core, street medicine is about building trust, honoring dignity, and creating consistent access to healthcare for people who are often excluded from traditional systems.
Our Program provides:
Basic First Aid: Immediate care for wounds, infections, minor injuries, and common illnesses to prevent complications.
Wellness Checks: Vital sign checks, symptom evaluations, and referrals for more serious conditions.
Infection and Illness Prevention: We strive to find wounds and illnesses early, helping to prevent avoidable hospital visits and serious health crises by connecting people to higher care.
Harm Reduction: Naloxone (Narcan) distribution, overdose education, and support for people at risk.
Preventive Care: Health education, hygiene kits, and supplies to prevent injuries and illness from even happening.
Disaster Relief: Emergency targeted outreach before, during and after extreme weather events, providing first aid, critical supplies and access to resources.
Compassionate Presence: Building trust and a human connection with those who are often overlooked by traditional healthcare systems.
How We Work?
Our teams are trained in basic life support and up to date street medicine practices, trauma-informed care, and local resources to best assist our clients.
We conduct regular street rounds, mobile outreach visits, and targeted response during emergencies.
We collaborate with schools, shelters, outreach groups, and public health agencies to extend the reach of care.
By addressing wounds, infections, illness and injuries early, we help people stay healthy and avoid unnecessary hospitalizations. Street medicine isn’t just about immediate treatment—it’s about long-term health, dignity, and consistent, compassionate presence.
