Each school year, you will need to complete a student health form for each of your students. Please print and bring completed copies to Open House if you desire the "Fast Pass " option. Printed copies will be provided at the school if you do not have access to a printer.

If your student needs to carry an inhaler, epi-pen, or insulin, you must complete these documents with your doctor so that they can do so. This would allow them to carry these important medications on field trips as well.

If your student has medicine that they need to take during the school day, you will need to complete this form with your doctor. This form would also allow your child to take these important medications during an overnight field trip, such as Camp Jekyll or Rock Eagle.
