Important: Physical Form Submission Required for Athletic Participation

Dear Students and Parents,

Please be advised that in order to be eligible to participate in any athletic activities, you must complete and submit the updated physical form.

Instructions:

1. Print the Physical Form – The form must be printed out in its entirety.

2. Complete and Sign – All required pages must be fully completed and signed by both the student and a parent or guardian.

3. Doctor’s Signature – Be sure to obtain all necessary physician signatures where indicated on the form.

4. Return Completed Forms – Once the form is complete, please submit the documents to the Athletic Department or the sport-specific head coach.

Important:
Students are not eligible to participate in any athletic activities until the updated physical form is submitted and cleared by the Athletic Department.
