This form is necessary if your child is scheduled for a BASE session and needs to attend clubs, tutoring, or activities that operate concurrently with BASE.
This form is required to be on-file for each designated/ approved pick up who between the ages of 16-18 years of age! You MUST communicate wiht the site-manager prior to submitting this form.
This form is necessary in order for BASE to accept your students inhaler and/or spacer at our prorgam. You MUST aslo provide your students medications in their orginal pharmacy box/ packaging WITH these forms. The medications must be handed off by the parent/ gaurdian to the prorgam Manager. You MUST speak with the program manager regarding your child's medications before they may attend the program.
