Off-Site Participation Form

Outside of our chapter, members can participate in other activities to help them meet their requirements. Please make sure participation falls into the following categories: Dance Instruction, Dance Clubs, or a Dance Team. Live Performances involving dance, Dance Workshops, Stage Crew, Sound or Lighting Crew, Set Design/Prop Crew, Choreography, Dance Captain/Rehearsal Assistant, Make-up or Costume Crew, Playbill Design, Photographer or Videographer.
