1. Read over the Student-Athlete/Parent Handbook and the FAQs section with your parents.
2. Go to the "Team Pages" tab at the top of this page and find the team you want to try out for and fill out the sign up form once it becomes available (usually a couple weeks before the start of the season).
3. Fill out an Athlete Participation Form (link below) and hand it into your coach once you have been informed that you are on the team. This form must be submitted one week after orientation has finished in order to remain on the team.
4. Attend team orientation days.
5. Pay your team Athletic Fee. This is due one week after orientation has been held. Those failing to pay this fee by the deadline may be removed from the team. If more time is needed to make payment, please contact the Athletic Coordinator to arrange an alternate date.

Everyone that plays for a Dalton team (MS or HS) MUST have one of these signed and returned before they are allowed to participate in any games!
For students that participate on multiple teams, you just have to bring this form in once for the year!
Signing this document means that you AND your parents have read and agreed to the terms and conditions listed in the Student-Athlete/Parent Handbook
If your son or daughter should be injured while participating in a high school or middle school sponsored or supervised interscholastic athletic event, the following procedures must be followed:
Click on the link above and fill out the Injury / Accident Report Form within 3 days of the injury
For insurance claims, pick up a claim form at CDS and return it within 3 days of the injury
See a physician within 3 days of the injury
CDS will then notify the parents of the process following this period.
PLEASE TAKE NOTICE THAT, IF YOU HAVE QUESTIONS ABOUT COVERAGE OR PARTICULAR TREATMENT UNDER THE SCHOOL’S ATHLETIC INSURANCE POLICY, YOU SHOULD INQUIRE ABOUT SUCH COVERAGE BEFORE YOU REQUEST OR AUTHORISE SUCH TREATMENT FROM A PHYSICIAN.
