Meningitis Information for Incoming 6th Graders 

6th Grade and New Transfer Students: 

The Clinton Township Board of Education requires a physical examination for all sixth grade and transfer students. The exam must be dated within one year of entry into school and needs to be documented on the NJ State Annual Physical Exam Form. A current immunization record should be submitted with the physical exam. Forms are available on this website.

If your child will be participating in sports, please refer to the "Sports Forms" tab to obtain those forms as well.

If you do not have health insurance, a link to NJ FamilyCare is available under the "Resources and Important Information" tab.

6th Grade Physical Forms are due in the Health Office by August 1st

6th GRADE STUDENTS CANNOT START SCHOOL UNTIL PHYSICALS & IMMUNIZATIONS HAVE BEEN SUBMITTED
