CTMS Health Office

Welcome to the Clinton Township Middle School Health Office web page. We would like to inform you of the health office requirements and assist with any health care needs your student may have during their educational years at CTMS. Our office is located on the first floor to the left of the main office.

Please keep us informed of any medical concerns that may arise throughout the school year. We look forward to working with you to provide a safe and healthy school experience!

Please submit all Sports Physical, immunization and general health forms for the 2024-25 school year by the first day of school by dropping it off to the CTMS Main Office and/or contacting the school nurse:
