Registration

Step 1: Fill out the Student Registration Form. Due Friday, November 17th.

Step 2: Have your Parent/Guardian fill out the Waiver Forms. Due Friday, December 15th

STEP 1: MESA Student Registration form

Fill out this Registration Form to begin the registration process. Once you have submitted your Form, you can move to Step 2. 

STEP 2:  Parent/Guardian Packet

Once the student has registered, a Parent/Guardian must fill out the waiver forms here.
