Kindergarten Entrance Immunization Guidelines
The Missouri Department of Health requires the following immunizations for all kindergarten students prior to the 1st day of school.
Measles, Mumps, Rubella: Two doses are required. One dose must be received on or after the first birthday. The second dose must be administered prior to beginning kindergarten. At least 30 days must separate the two doses.
Polio: At least 3 doses of Polio are required, with the last dose received at age 4 or older. No more than 4 doses of Polio Vaccine need to be given to children less than 7 years of age. Vaccine (TOPV) – same as above.
Diphtheria: (DTaP, DPT, DT, Td): At least 4 doses, with the last dose received at age 4 or older. No more than 6 doses of Diphtheria vaccine should be given to children less than 7 years of age. An additional dose of Tdap is required 10 years after the last dose.
Hept. B: 3 doses are required for all kindergartners. Initial visit 1 dose, 1 month later, 2nd dose, and 6 months after 1st dose you should get the 3rd dose. If the Hept. B series has been interrupted; there is no need to restart the series. Just go ahead with the next dose. Must be at least 24 weeks or 168 days. Child must be at least 6 months of age by the date of the 3rd dose.
Varicella: 2 doses of (chicken pox) vaccine required or written statement signed and dated with mm/dd/yr of illness; must be obtained from a physician. Parental or guardian statements of disease will no longer be accepted.
*If immunizations are not up to date by the entrance into school, the child will be excluded from school until the immunizations are given.*
Exceptions:
A child may be exempted from immunizations for Medical Reasons by placing a Physician’s Medical Exemption form on file with the child’s school immunization record. A Physician must sign this. This does not need to be renewed yearly.
A child may be exempted for reasons of Religious objection by placing a Religious Exemption form on file with the child’s school immunization record. This must be signed and renewed annually.
Students who are incompletely immunized may continue to attend school as long as they are making satisfactory progress as outlined on the immunization “In Progress Form” (which must be on file with the child’s immunization record).
*A copy of updated immunizations can be emailed to tiffani.shuman@cr6.org, brought to the office secretary, or faxed to the school. Copies need turned into the office prior to the first day of school. You may begin emailing copies of the immunization to Dr. Shuman. Please note your child's full name in the email.
