Sports Physical 

A sports physical is MANDATORY BEFORE tryouts and/or practices 

There are two parts to the CPS sports physical:

• Players packet --> information about you signed by a parent or guardian

• IHSA physical (inside players packet) --> to be filled our by doctor

  • This can be a family doctor or by someone at an intimidate care clinic
