Incident Reporting

Cook Unity utilizes an anonymous incident reporting tool called Spot!

Spot will help us surface issues early and manage them without compromising anonymity. A few things to know:

•  No one will see what you chat with Spot about unless you decide to submit a report

•  Spot may ask for your email address so it can act as a go-between

•  No one at Cook Unity, including HR or leadership, can see that email address

•  You can document issues that involve you directly or things that you witness

•  If you want to disclose your identity, you can

• We encourage everyone to check out Spot now so you know where to go if something happens.
