A Power of Attorney is an official document that gives someone else the right to act on your behalf. It should be signed by all relevant parties in front of a Notary Public.
Colorado Parental Minor Power of Attorney (form) - for the day-to-day care, custody, education, recreation, property, and medical care of children; a copy should be shared with the school.
