Student Health Requirements
Dear Parents and Guardians,
We hope this letter finds you well as we prepare for a successful and healthy school year ahead. In accordance with the Pennsylvania Department of Health regulations, all students are required to meet specific health requirements, including physical and dental examinations as well as up-to-date immunizations.
Please review the following requirements carefully to ensure your child is in compliance for the 2025–2026 school year:
All newly enrolled students—regardless of grade level—are required to submit documentation of their physical exam, dental exam, and immunization records.
Kindergarten
1st Grade
6th Grade
11th Grade
You may submit a physical completed within one year before the start of school. A licensed healthcare provider must sign the form.
Kindergarten
1st Grade
3rd Grade
7th Grade
Dental exams must also be completed within one year before school entry and signed by the dentist.
4 doses -DTaP (Diphtheria, Tetanus, Pertussis)
4 doses -Polio
2 doses -MMR (Measles, Mumps, Rubella)
3 doses -Hepatitis B
2 doses -Varicella (Chickenpox)
7th Grade students also require:
1 dose of Tdap
1 dose of Meningococcal (MCV4)
12th Grade students require:
A second dose of Meningococcal (MCV4) if the first dose was before age 16.
Students who are not in compliance with immunization requirements may be excluded from school until proper documentation is submitted, per state law.
