As we work together to support the safety and well-being of every child at CHP Schools, we kindly remind all families to keep the school informed of any medical conditions, medication needs, or changes in your child’s health throughout the school year.
Having the most accurate and up-to-date health information helps our School Nurse and staff respond quickly and appropriately in the event of illness, injury, or emergency.
Has a new diagnosis (e.g., asthma, high blood pressure, sprain, broken bone, etc.)
Starts or stops any prescription or over-the-counter medication
Has a change in dosage or medication schedule
Needs an emergency care plan (for asthma, seizures, anaphylaxis, or any other critical illness)
Has had a recent hospitalization, surgery, or ongoing medical treatment
Develops any condition that may affect participation in school activities
Contact your School Nurse with any updates.
Complete any necessary health forms (available on our website or from the nurse’s office).
Work with your child’s doctor to provide care plans or medication orders, if needed.
Your communication helps us:
Support your child’s health needs safely and respectfully
Ensure medications are administered properly
Alert trained staff to respond to emergencies
Foster a learning environment where all students can thrive

Proper documentation and preparation help us support your child’s health and safety throughout the school day. We ask that families submit the appropriate medical forms and supplies for any known health conditions.
Anaphylaxis, Asthma, Diabetes and Epilepsy/ Seizures require specific documentation listed below.
If your child has a chronic or acute medical condition that is not included in this list, please contact your School Nurse to discuss what forms and equipment may be needed.
All required forms and medical supplies should be submitted prior to the first day of school or as soon as your child receives a diagnosis.
Required Asthma Documentation & Supplies
This form must be completed and signed by your child’s healthcare provider. It outlines steps for managing asthma at school, including symptoms, medications, and emergency instructions.
Medication Authorization (Form MED-1)
If your child uses an inhaler or nebulizer during the school day, a completed MED-1 form is required from the prescribing provider.
This form allows the school nurse to administer medication or treatments and outlines how and when it should be given.

This form must be completed and signed by your child’s healthcare provider. It outlines steps for managing severe allergic reactions at school, including symptoms, medications, and emergency instructions.

Diabetes Medical Management Plan (DMMP)
This form must be completed and signed by your child’s healthcare provider.
This is for all students with diabetes both Type 1 and Type 2
It provides essential guidance for managing your child's diabetes while at school.
Medication Authorization Form (MED-1)
If your child uses insulin, glucagon, or any prescribed diabetes-related treatment, a MED-1 form must be completed by the prescribing provider for each medication/treatment.
This form allows the school nurse to administer medication or treatments and outlines how and when it should be given.

All necessary equipment must be brought to school clearly labeled with your child’s name
Glucose meter (One to be kept in the Nurse's office) with appropriate lancets and test strips
Insulin pens and needles or

Snacks for low blood sugar
Ketone test strips
