Dear Parent or Guardian,
As part of our commitment to student health and safety, CHP Schools follow medication guidelines aligned with the Pennsylvania Department of Health regulations. This letter outlines important information about how medications must be handled during the school day.
If your child needs to take or use any medication during the school day, whether prescription or over-the-counter (OTC), please follow these steps:
Medical Authorization Form
A Medication Administration Form (MED-1) must be completed and signed by:
Your child’s licensed healthcare provider (with dosage, schedule, and condition), and
You, the parent or guardian.
Medication Labeling Requirements
Prescription medication must be in the original container with the pharmacy label.
OTC medication (such as Tylenol, Motrin, etc.) must be new, unopened, and clearly labeled with your child’s name.
We cannot accept medication in baggies or unlabeled bottles.
Medication Delivery
A parent or guardian must bring the medication directly to the school nurse.
Students may not carry medication unless specifically authorized (see below).
Medications sent to school without the appropriate paperwork will not be administered.
Students may be allowed to carry and self-administer the following emergency medications with written permission:
Asthma inhalers
Epinephrine auto-injectors (EpiPens)
Diabetes care supplies (with care plan)
To allow this, the school must receive:
A Self-Administration Authorization Form
A note from your child’s provider confirming they are trained and capable
A signed parent/guardian consent
These documents must be reviewed and approved by the school nurse annually.
For virtual students participating in school-related trips or in-person activities, a MED-1 form is still required unless a parent or guardian will be present for the entire duration of the event.
If you believe your child may only require medication during field trips or special school activities a MED-1 form is still required.
No student may carry or use medication without prior approval.
Unauthorized medications may be confiscated, and parents will be contacted.
Please submit all required documents before the first day of school or as soon as medication needs arise.
