Annual Electronic Forms
Yearly forms are now digital. Please make sure to review all forms and complete the electronic sign-off where appropriate. Please click HERE to acces steps for completing the electronic forms. On the first day, your child will receive a paper packet containing the Green and White Calendar, an Important Dates document, and a Parent Information resource with steps on how to access and update your forms. Please know that it is very important that we have all current contact information as well as your emergency contacts.

Effective School Solutions (ESS)
ESS is a partner of the Chariho Regional School District that provides student mental health supports. As our partner, they will provide five (5) virtual workshops to support parents and caregivers that want to be informed about student mental health needs across the '25-'26 School year.
Interested parents and caregivers: Please complete this short Google form so we can ensure that ESS prioritizes the virtual workshop topics with the most community interest.
