Benefit enrollment and changes take place in the Infor self service portal.
Infor can be accessed from home or at work. If you are unfamiliar with Infor, please check the Infor Resources Page for all Infor related topics and How To's.

Who is Eligible for Benefits?
You are eligible for benefits if you are an employee budgeted and scheduled to work at least 20 hours weekly.
Temporary employees are eligible for some of the offered benefits.

If you miss the enrollment deadlines identified below you must wait until the next annual open enrollment or experience a qualifying event to enroll in or make changes to your benefits.
New Hire

30 days from Date of Hire to complete and submit enrollment
Benefits are effective 1st of the following month
For CHAPO staff, Health and Dental benefits are effective day of hire.

Domestic Partner: coverage is subject to Imputed Income & requires a Domestic Partner Declaration/Affidavit
Exception: Cambridge Payroll Dental/Vision plan does not allow Domestic Partners

Medical & Vision: Up to age 26
Dental: Up to age 26
Exception: Cambridge Dental/Vision Plan (weekly employees only) allows up to age 26 with specific requirements noted here. This plan also requires a paper form for enrollment and any changes.

Birth Certificate/Adoption/Proof of legal guardianship (children)
Domestic Partner Declaration/Affidavit (domestic partner)
Open Enrollment happens each fall. You will see an Open Enrollment tab during Open Enrollment with details.
Open Enrollment changes are effective on January 1 of the following year. Open Enrollment is your only chance to make changes outside of a qualifying event.
Information for upcoming Open Enrollments will be mailed to your home address and emailed to you as Open Enrollment approaches.
