Exclusive Provider Organization – EPO
CHA PCP selection required
Narrow network
Must utilize CHA providers and facilities for your care (narrow exceptions)
Not available to employees on the Paychex payroll
A selection is required to complete enrollment in Infor for health insurance.
If you do not wish to enroll in a CHA Health Plan, you will select the Waive option.
If you are in a union and have non-CHA health coverage, you may be eligible for the waiver option with payout. With proof of other coverage (a copy of your insurance card), you can receive a waiver benefit! Check your union contract on Staffnet or your Benefit Summary for eligibility and amounts.
If you have a disabled dependent who otherwise wouldn't be covered, you can submit a Disabled Dependent Application directly to HPI for approval. They will notify us if the application is approved and we will add the dependent to your plan and if needed, change you from individual to family coverage.
CHA offers many locations and departments to ensure you have access to the care you want and need.
If you have the CHA Option Plan, you must choose a CHA Provider.

With the Choice Plan, you are enrolled in the plan, NOT a specific tier.
If you have the Choice Plan, you can find a provider by visiting www.healthplansinc.com/cha.
Where you receive care (and what tier that falls into) determines how the claim will be processed.

You will receive your physical ID card by mail within 14 business days of the effective date of your health insurance.
You can also download your ID card online. You card will be available within 14 days from your finalized email.
