ECMS Band Student/Parent Information Form

The ECMS Band Information Form must be completed for ALL ECMS Band students ate the beginning of each school year to ensure that Mr. Walavich has all of the correct information for the band students.  Students who join band later in the year should complete this form as soon as possible.  Please take a moment to double check the information you include so that it is completely correct.  Thank you in advance for your help! This will ensure that students and parents are getting all of the correct communication throughout the year.
