Each grading period

Accommodations:

Mark language accommodations you provided each marking period.
If no accommodations were needed make sure to check the top gray box.
SIGN and DATE at the bottom for current MP

Grade Tracking:

If any EB student fails a class for a grading period you will need to fill out this google form.
