Complete using the following timelines:
Local (w/in OC) = 3 calendar weeks in advance
Out of Area (out of OC but w/in CA) = 6 calendar weeks in advance
Out of State/Country = 2 months in advance.
If using ASB Funds, verify available funds with Activities Accounts Clerk.
When approved and estimated costs are provided, SUBMIT P.O. REQUESTS in order to encumber funds - do this BEFORE you are invoiced (i.e. - transportation bills)
Complete ALL required forms
Field Trip Forms Folder - Complete and submit the forms found to Lisa Bowen in the Main Office
ASB Paperwork
Collect any/all monies from involved groups PRIOR to the field trip
ALWAYS issue a receipt (or student signed Activity Group Receipt) for collected monies
Establish timelines to ensure that all monies are collected for payment to venue, transportation, etc.
Work with the school nurse at least 2 weeks (minimum) prior if you are taking students who are not on your roster. A 2-week minimum is STRONGLY RECOMMENDED.
**For overnight trips, notify the nurse 4 weeks prior to the trip.
Day of trip – See School Nurse to coordinate medication pick up (directions regarding doctors orders and medications needed: epi-pens, inhalers, etc.)
Upon return to school, return medications to the school nurse:
The day of if returning prior to the end of school day
Return to the nurse the next school day if returning in the evening, Friday night or a weekend day.
Ensure attendance is taken prior to students leaving campus.
Provide a copy of this attendance list the attendance office before leaving the school.
Advisers/coaches CANNOT authorize or grant permission for students to ride with another student’s parents to school related activities and events.
Keep copies of permission slips & Medical Release/Liabilities forms with you at all times during the trip.
Be sure there is a CERTIFICATED staff member present during the trip at all times.
