Residency Form

This is a special Proof of Residence form that must be completely filled in and notarized. The Residency form is used when the student and their parent/guardian live with another person, and all utility bills are in that person's name. Use of this form does not guarantee enrollment at the requested campus. If this form is used for Proof of Residence and the living situation changes, the parent/guardian must contact the campus and provide a new proof of residence. Should this form be used for Proof of Residence and the living situation is found to be different, enrollment may be revoked by the campus admin and/or district office.

A Residency Form is used when you and your student are currently living with a Canyon ISD resident whose address is located within the district's attendance boundaries.

If this reflects your current living situation, you will need to provide the following:

• A completed and notarized Residency Form.​ Canyon ISD Residency Form (click link)

• The Canyon ISD resident's current proof of residency.

• A current copy of the Canyon ISD resident's valid driver's license or government-issued photo ID.

• A current copy of your valid driver's license or government-issued photo ID.

Please understand that Canyon ISD takes residency verification very seriously. All residency information is subject to review and verification, and the district reserves the right to deny any Residency Form or supporting documentation that is determined to be incomplete, inaccurate, or falsified.

If you are not currently living at the Canyon ISD residence, we encourage you to wait until your move has actually taken place before submitting a Residency Form. Providing false residency information may result in your student being immediately withdrawn from their current campus and returned to their zoned school.
