Grandparent Form

This is a special Proof of Residence form that must be completely filled in and notarized. This form is used when the student and their parent/guardian live outside CISD/campus boundaries, but the grandparent lives inside CISD/campus boundaries and will pick the student up 4 or more times each week. Use of this form does not guarantee enrollment at the requested campus. Use of this form removes the student's ability to ride the bus to or from school. Use of this form removes the student's ability to attend the CISD Kids after school program. If this form is used for Proof of Residence and the living situation changes, the parent/guardian must contact the campus and provide a new proof of residence. Should this form be used for Proof of Residence and the living situation is found to be different, enrollment may be revoked by the campus admin and/or district office.
